• Enter information about CCC and how many people you think will participate
• How funds will be used statement: These funds will be used to support Coalition to Cure CHD2's mission: to improve the lives of those affected by CHD2-related disorders by increasing education, building community, and accelerating research to uncover a cure.
• Don't forget to check the boxes next to the legal agreements and reCAPTCHA

Step 5

While your application is being reviewed, you will receive a message from Chipotle where you will be asked to enter:

• CCC's EIN: 85-4057370
• Memo Line for Check: [your last name] - name of your fundraiser*
• Check Payee Name: Coaltion to Cure CHD2, Inc.
• Mailing Address for Check: 320 E Ogden Ave, Naperville, IL 60563
